You should speak to your doctor if you have one of the listed conditions to qualify for the program. It would be best if you found another doctor if you are unable to register with your doctor. However, you have one of the debilitating conditions listed in the program.
A physician can recommend participation in the program, but there is a registration process. Patients must complete the registration process initiated by their physician. A link is then sent to the patient via email which links them to a Department of Consumer Protection website called Biznet. Here is where the patient creates a new account with the state and completes their process by uploading a proof of identification (like a drivers license), a proof of residency (like a cellphone bill), a selfie on a plain white background, and a $100 registration fee (this must be renewed annually). There is a short processing time with the state (not including weekends or holidays) in which you will receive your valid card via email. Those renewing get it via email in about 30 seconds after paying.
Curious… Why are the doctors not registering patients? Like in ANY other state? For what some of these guys are charging patients they should be wrapping up the whole process for them.
